Your patient can contact us directly to schedule an appointment. We will evaluate the patient and send a follow-up letter and detailed script back to the patient’s medical provider.

Outside Our Facility

We can also see your patients during inpatient stays at hospitals and rehabilitation facilities. For homebound patients, we can arrange home visits on a case-by-case basis.
